- Copy of Mortgage Statement or Rental Agreement
- Copy of Payment Ledger or Payment History
- Copy of utility bill (can be electric, gas, or water)
- Completed MIL Screening Form (you can find below)
The following bills are NOT approved for assistance:
- Auto repairs
- Cable
- Eviction
- Funerals
- Insurance
- Late fees
- Medical
- Storage
- Telephone
All cases are determined on a case-by-case basis
