- Copy of your most recent pay stub
- Valid Drivers License or Government Issued ID
- Copy of Mortgage Statement or Rental Agreement
- Copy of Payment Ledger or Payment History
- Copy of utility bill (can be electric, gas, or water)
- Completed MIL Screening Form (you can find below)
